Ridgeview Community Network is an accountable care organization (a network of physicians, hospitals, clinics and health care providers that leverage the full care system to ultimately improve quality and patient outcomes and reduce costs to both employers and their employees). Ridgeview Community Network provides local, personalized care at one of the few independent health care systems in Minnesota with partnerships including hundreds of other providers and top specialists from across the Twin Cities. More than 300 physicians and nearly 2,000 medical and support staff deliver coordinated care locally within 37 primary care clinics and more than 150 specialty clinics throughout a seven-county west metro area and beyond. No referral is needed to see a participating Ridgeview Community Network provider. Members have direct access to Ridgeview Hospitals and Clinics; Ridgeview’s Waconia Campus; Ridgeview’s Arlington Campus; Ridgeview’s Le Sueur Campus; Lakeview Clinic, Ltd.; OBGYN West; Wayzata Children’s Clinic; Western OB/GYN, A Division of Ridgeview Clinics; South Lake Pediatrics, and Catalyst Medical Clinic. In addition, Ridgeview Community Network partners with six hospitals including Ridgeview’s Waconia campus, Ridgeview’s Arlington campus, Ridgeview’s Le Sueur campus, Abbott Northwestern Hospital, Hennepin County Medical Center and all of Children’s Minnesota – including specialists located in Ridgeview’s Emergency at Two Twelve Medical Center in Chaska.
